Family Pathways provides information for older adults and their families. Family Pathways is a place to turn to for guidance and advice during difficult times.
The Community Advocates of Family Pathways help connect individuals and families with beneficial services, such as financial assistance, transportation, and transitional housing. Along with this, the Community Advocates help people determine the root cause of their immediate crises and develop long-term plans for self-sufficiency.
Community Advocates provide the following services:
Perform home assessments to identify need.
Assist with creating a core plan for the future, including community, government, private and family resources.
Demonstrate how to engage other community resources
Assist family members, friends and neighbors in creating a "Circle of Support."
